Advances in digital technologies are rapidly transforming healthcare delivery, and artificial intelligence (AI) is poised to reshape the self-management paradigm for older adults living with chronic conditions. This seminar introduces nursing research that leverages digital technologies to support chronic disease self-management, approached through the lens of balancing digital innovation with human-centered nursing and care. The study “A Tailored Digital Health Literacy–Based Remote Self-Management Program for Community-Dwelling Older Adults Living Alone” highlights how digital care extends beyond technological intervention by demonstrating its applicability as an approach grounded in human-centered nursing, self-care support, and expanded service delivery. Another project—“Development of a Self-Management Program for Older Adults With Diabetes Using Social and Digital Determinants of Health and Generative AI”—explores how generative AI–driven conversational feedback and personalized health coaching can strengthen self-efficacy and promote self-management behaviors. Through these examples, the presentation proposes a co-care system in which AI agents and nurses collaborate and seeks pathways to harmonize the core values of nursing with digital innovation.
